All rights reserved. Introduction Leiomyosarcoma of the penis (LSP) is an extremely rare form of penile tumor. LSP can be divided into two subtypes: deep and superficial. The goal of this paper is to present a very rare case of LSP. Case outline On examination, the patient presented with a slowly “growing penile bump”, for which an initial diagnosis of non-inflamed penile atheroma was given. Further diagnostic workup was omitted. Outpatient excisional biopsy was performed, and the tumor was sent for pathohistological examination, which revealed penile leiomyosarcoma. The patient has not received any further treatment. The most recent follow up was two and a half years after surgery, and the patient continues to do well without any complaints. Conclusion LSP is an extremely rare disease, which can be cured if it is diagnosed in its early stage.
